Facial asymmetry is a common yet complex challenge in orthodontic diagnosis and treatment planning. An increasing number of cases, particularly those associated with uneven condylar size, are now detected on panoramic radiographs during routine examinations. A key clinical question remains: Should treatment begin now, or should we wait? This session will focus on identifying cases that seem stable but may continue to progress after growth has finished.

We will review updated diagnostic approaches, including clinical evaluation, 2D imaging, and the expanding role of CBCT and 3D superimposition for detecting skeletal discrepancies. The role of bone scintigraphy in assessing active condylar growth will also be discussed. Emphasis will be placed on current gold-standard assessment methods and practical strategies for integrating these tools into daily practice.

The session concludes with real case examples, offering guidance on evaluation, timing, and treatment planning — from deciding when to observe to determining when intervention is necessary. Collaboration between orthodontists and oral and maxillofacial surgeons will be highlighted as essential for optimal management.
